Download, print, and complete the Passport Application Form DS-11. 

Your departure date, if known, and your Social Security number must be included on the application. It is important that you fill out all the required information on the application. Do not sign the application until you are instructed to do so in the presence of the Passport Agent who is processing it. 

You must submit one (1) of the following to provide proof of U.S. Citizenship:

• A prior U.S. Passport or Passport Card
• U.S. Birth Certificate that meets the following requirements:
  • Issued by the city, county, or state of birth
  • Lists full name, date of birth, and place of birth
  • Lists parents’ full names
  • Has the raised, embossed, or multi-colored seal of the issuing authority
  • Has the date filed with the registrar’s office (must be within one year of birth)
  • Has the registrar’s signature

If Born Outside of the United States:

• Consular Report of Birth Abroad or Certification of Birth
• Certificate of Naturalization
• Certificate of Citizenship

Please note, original documents are required, photocopies and notarized copies are not acceptable.

You must present one (1) of the following identification documents to the acceptance agent. The ID must readily identify you.

• A valid, driver’s license. Out-of-state driver’s licenses will require additional identification.
• A valid or expired, undamaged U.S. passport book or card
• Certificate of Naturalization
• Certificate of Citizenship
• Government Employee ID (city, county, state, or federal)
• U.S. military or military dependent ID
• A valid foreign passport

If you do not have a Primary ID as listed above, you may present at least two (2) Secondary forms of identification. For a list of acceptable IDs please visit the U.S. Passports and International Travel Identification Page. 

The application fee and the execution fee are paid separately. The federal processing fee (application fee) must be paid via check or money order made payable to the U.S. Department of State. The County processing fee (execution fee) can be made by cash, credit card, or check made payable to the Middlesex County Clerk.

If traveling in less than six weeks, we recommend passports should be expedited. That fee is an additional $60 paid per application, in addition to the required fees. This will ensure faster processing than the routine service. 

All minors must appear in-person for their passport along with an original long form birth certificate (with both parent’s name listed).

For renewal, the minor’s previous passport must also be presented. 

Passports issued for children under the age of 16 will require the presence of both parents, unless:

• Submit a notarized U.S. Department of State consent form DS-3053 from the non-appearing parent as evidence of sole authority to apply
• Submit a court order showing sole custody of the child
• Submit a death certificate if a parent is deceased
• Only one parent is listed on the birth certificate

Applicants aged 16 and 17 must be accompanied by one parent.
