Diabetes Self-Management and Support Program
The Middlesex County Office of Health Services Diabetes Self-Management and Support Program is a free, accredited program designed to provide tools for people at risk of developing diabetes, or already diagnosed with type 2 diabetes. The program is flexible, self-paced, and includes activities and resources to help you live a healthier lifestyle.
- Patient-centered: We will listen to what types of support are most important to you.
- Flexible: We will meet you where you are and tailor interventions to meet your health and wellness goals.
Program Eligibility:
- At least 18 years old
- At risk for developing type 2 diabetes or
- Diagnosed with prediabetes or type 2 diabetes by a health care provider
Let our staff assist you in your journey of living with diabetes. Join one of our diabetes education sessions focused on nutrition, physical activity, healthy coping, and reducing risks.
This is an ongoing active program, and we are looking for residents who need help managing their diabetes.
Enrollment open to all residents.
