Special Child Health Services Case Management UnitThe Middlesex County Special Child Health Services (SCHS) program provides comprehensive Case Management for special needs children from birth to the 22nd birthday, who may have a disease, disorder or condition that may interfere with normal growth or development.

Case Management Services

The Case Management Services include a review of your child’s needs, an assessment of on-going services being received, exploring new ways to fulfill unmet needs and developing a comprehensive plan of care for your child.

The program offers resources and referrals to community, state and federal programs including but not limited to the NJ State Catastrophic Illness in Children Relief Fund Program. Upon referral, the eligible child is assigned a Case Manager, who with parental consent, completes an assessment and explores ways to meet the child’s needs. Medical, educational, developmental, social and economic needs of the child and family are targeted.
