Take Control of Your Health
Many Middlesex County residents live with diabetes, pain, and other chronic conditions and diseases. The County offers several self-management education workshops that aim to teach individuals and caregivers necessary skills to manage chronic conditions, such as goal setting, problem-solving, handling difficult emotions, eating well, and exercising safely and easily.
Participants attend weekly sessions, held for 2 ½ hours, for six weeks. In Middlesex County, these programs are delivered in various community settings such as recreation centers, libraries, health centers, faith-based organizations, and senior centers throughout the year.
